Welcome to FCA where you will complete the Psychological component of your assessment process toward Deacon Formation. My name is Tim Heck and I am a Licensed Marriage and Family Therapist with FCA. It is my hope that through the completion and discussion of several questionnaires by you and your spouse, as well as 90 minutes of your time in a face-to-face Interview, I am able to provide a thorough, formal Psychological Assessment Report that accurately portrays you and your marriage relationship. The ultimate objective is to provide another dimension of the assessment for the Church leadership to effectively discern for your participation and acceptance in Deacon Formatio

THE THREE-STEP PROCESS

STEP TWO: Call the FCA Main Office to schedule your Appointment to meet with Dr. Tim Heck.

You can reach the FCA Main Office at (317) 585-1060, Press "0" when you hear the main menu. Or you can use our Toll-Free Number - (888) 701-1060 and Press "0" when you hear the main menu. You will be scheduling a One-Hour Appointment with Dr. Heck or a 90-Minute Appointment if you are Married. If you are married, your Spouse will need to accompany you to the Appointment and will participate in the last 30 minutes of the Interview with Dr. Heck. DIRECTIONS

STEP THREE: Complete the following three Tests.  Please note: Wives must complete all of the tests, as well as the Deacon Applicant.

We need you to complete several online Tests. Please click on the links below, one at a time, complete the Test, then return to this page to proceed to the next Test. All results are confidential and the information will only be viewed by your Therapist, Dr. Heck, who will then incorporate the results into your Interview with him in coming weeks and the Final Psychological Assessment Report to the Diocesan Deaconate Office. For the first test, the T-JTA, you and your Spouse (if Married), will both take the test. After you complete the test, please have your wife start the process over and take the T-JTA. Please do not compare your responses and complete the test independently.

Our FCA Office will send you an E-Mail with your USER NAME and PASSWORD to enable you to complete the 16 Personlity Factor Test (16PF).

In addition, there will be two Tests you will be asked to complete when you arrive at our Office for the Interview Appointment with Dr. Heck. Please plan an additional hour to complete these Questionnaires either before or after your Interview Session with Dr. Heck.
